Financial Information

In order to build a trustworthy physician-patient relationship, your clear understanding of our financial policy is important. We will discuss with you the cost of your treatment and each of your available payment plan options before you begin treatment so that you can make the best choice for you, your smile, and your family. As a part of a professional relationship, we kindly ask that you make your payment at the time service is rendered. For your convenience, we accept cash, checks, credit cards, or CareCredit card.

The charge of a returned check is $25 payable by cash or money order. This will be applied to your account in addition to the insufficient fund amount. 

Insurance Policy

Insurance is a contract between you, your employer, and your insurance company. Although we may estimate what your insurance company may pay, it is the insurance company that makes the final determination of your eligibility and benefits. If you have any dental and /or medical insurance, we will gladly help you fill out the paperwork submit your claims for you and will make every effort to work hand in hand with you to maximize your insurance reimbursement for covered procedures. In order to properly bill your insurance company, we require that you disclose all insurance information including primary and secondary insurances, as well as, any change of insurance information. If requested, we will submit a pre-treatment estimate, to your insurance company, prior to your surgical appointment.

For your convenience, we are participating with the following dental insurance companies:

  • United Concordia: Tricare Dental Program, Active Duty Dental Program, Elite​ *We are opt out of all third party insurance companies utilizing United Concordia.
  • Delta Dental PPO and Premier

Insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. It is ultimately your responsibility to pay all fees charged by this office regardless of your insurance coverage.

Minors & Guarantors

All patients under the age of 18 years of age must be accompanied by a parent or guardian for all visits including surgery and post-operative visits. Regardless of who is the policyholder for the insurance, the parent or guardian present will be responsible for any amount owed.

Outstanding Balance Policy

It is our policy that all past due accounts be sent three notifications Returned checks and balances older than 180 days and no resolution can be made, the account will be sent to a collection agency and you will be responsible for all collections costs including attorney fees and court costs.
